Reimagining Systems of Care
"Reimagining systems of care" refers to transforming existing healthcare and social service models to be more effective, equitable, and person-centered by addressing systemic issues and integrating new approaches. These themes and the implementation of new information and frameworks require top down leadership, and most importantly, they require curiosity and participation from the public.
Key themes include ethical incorporation & use of technology, focusing on social determinants of health, adopting restorative and patient-centered values, improving working conditions, and fostering stronger community and patient partnerships to build cultural acuity, and more resilient and supportive systems.
Placing the individual at the core of the system, prioritizing their needs, and engaging them in their own care journey.
Restorative Approaches:
Emphasizing the strengthening of relationships between providers and patients/communities through shared community and individual values and dialogue, rather than solely transactional interactions.
Addressing Social Determinants of Health:
Recognizing and acting on factors like poverty, food security, education & literacy and access to resources that impact health outcomes.
Integration and Partnerships:
Breaking down barriers between different parts of the care continuum and collaborating across organizations to provide seamless, holistic support, fostering new frameworks of care in community public health.
Reimagining systems of care is not only a responsibility of those who work within the systems themselves, but also a responsibility of each member of the community. Through intentional interaction within existing systems, we actively build the future of community public health that best supports us, and the next seven generations.
Through understanding and recognition of the impact of individual mindsets, actions and habits on the greater collective, we can continuously work toward embodying our highest expressions in service to humanity. We are here to actively build and participate in the future.
Our highest potential is not only witnessed through positive transformation of global systems, but is observed and witnessed on a smaller scale: through the transformative process of human growth and development through the lifespan. The physical body provides cues of personal and collective consciousness: forward growth, expansion, alignment, and health. These themes are witnessed through the observation and assessment of the head and neck, face, teeth and jaws, inviting us all to see the vast potential of our-Self and encouraging us to reach those potentials, sometimes on our own, and sometimes by engaging in aligned relationship with care systems & practitioners who can lead us and help guide the way forward.
